As a note, I listened to the audio book, I do not own the text.
Primarily for difficulty this book presents a small amount of a medical vocab (the book takes place in a hospice) and the quirk of using 言霊 in places where normally one would say 言葉 or even 話す/言う. Otherwise the grammar and vocab are all pretty standard and I think many scenes are overwritten to the point that any confusion a learner may have would be cleared up by sheer repetition of the point from different angles.
