Primarily for difficulty this book presents a small amount of a medical vocab (the book takes place in a hospice) and the quirk of using 言霊 in places where normally one would say 言葉 or even 話す/言う. Otherwise the grammar and vocab are all pretty standard and I think many scenes are overwritten to the point that any confusion a learner may have would be cleared up by sheer repetition of the point from different angles.
On that note, I did find the writing of this book to be a bit brow-beat-y. I'm not entirely sure the intended audience as I know this author to primarily write for adults, but after many scenes that felt completely overwritten I think maybe it is geared towards a younger set? After I accepted it for what it was I was able to enjoy the story, but it's not his usual style so I found it a bit jarring at first.
Overall I'd describe this book as 癒し系 with 死神 for people who want a smidge more darkness/suspense in their story than is typical for the genre.
