This is my first older book, and everything I've read up to now caps out around ~31 difficulty on this site (barring some visual novels?), so I'm speaking from that perspective.
Definitely a very slow read for me, but not as bad as I expected. It's heavy on keigo, lots of kana words are in kanji, there are a few odd things that my yomichan dictionaries couldn't pick up on, etc. The worst of it comes in these long sentences of little clauses separated by commas that feel like they'll never end, but there aren't TOO many and they're a bit front-loaded. Over time its less common words start repeating and I got more or less comfortable, though I was looking something up in everything sentence basically. The actual grammar/complexity isn't that bad if you're around this level, and the Aozora version puts helpful furigana over a whole lot of things.
It's a great story! I seemed to find it less creepy than most people do, but I always looked forward to reading more and love what a unique little thing it is. Highly recommend.
